The human spine is strong, flexible, and a little bit of an evolutionary compromise. We are the only animals who habitually stand up and walk on two legs. While there are advantages of this arrangement -- keeping our hands free to manipulate objects, and our jaws free to speak, there is a price, and that price is back pain. Our spines were simply not designed to support beings as upright as we've become. The spine can form unnatural curves, twists, or break. The discs between the vertebras can wear out. All of these can ultimately cause pain.
